Fall Protection Requirements for California Construction

Between 2015 and 2026 M Squared Safety evaluated 403,323 individual inspection items across 7,473 job site inspections in California. Fall protection is the worst performing large category we measure, at 18.5 percent across the two fall protection sections of our inspection form. Inside it, one hazard fails on three separate counts.

The hole in the floor

Three of the items we check are about the same thing: an opening in a walking surface. All three fail at rates nothing else in our record approaches.

  • Floor hole covers secured and marked: 41.1 percent (39 of 95)
  • Floor holes adequately guarded or covered: 39.6 percent (55 of 139)
  • Floor hole covers adequate and substantial: 26.8 percent (22 of 82)

A guardrail is visible. Somebody notices when it is missing. A hole in a deck with a loose sheet of plywood over it looks finished, and the sheet gets picked up by the next trade who needs a sheet.

Fall protection overall: 1,017 findings across 5,511 checks, 18.5 percent, combining the general and roofing fall protection sections of our form. That is more than three times the all-category average of 5.1 percent.

The marking requirement almost nobody meets

8 CCR 1632(b)(3) requires that covers be secured in place to prevent accidental removal or displacement, and that they bear a pressure sensitized, painted, or stenciled sign with legible letters not less than one inch high, stating "Opening--Do Not Remove".

The words are part of the standard. So is the letter height. A cover that is screwed down and blank fails the second half of the sentence.

The same subsection sets the strength: covers must be capable of safely supporting the greater of 400 pounds or twice the weight of the employees, equipment and materials that may be imposed on any one square foot area of the cover at any time. A single sheet of half inch plywood over a large opening does not meet that, which is what the 26.8 percent adequacy failure is measuring.

California triggers at 7 1/2 feet, not 6

8 CCR 1670(a) sets the general construction trigger at falls in excess of 7 1/2 feet from the perimeter of a structure, unprotected sides and edges, leading edges, through shaftways and openings, and sloped roof surfaces steeper than 7:12.

The federal construction standard triggers at 6 feet. Most fall protection content published online quotes the federal number, because most of it is written for a national audience.

Read that carefully before acting on it. 7 1/2 feet is the general trigger in 1670(a) only. Several other California sections require protection at lower heights for specific operations, and the correct answer on a given job depends on which section governs the work. This is the opposite of the fire extinguisher case, where California is stricter than federal and out-of-state contractors get caught short. Here the numbers run the other way, and assuming the looser number across the board is how people get hurt.

What failed, how often, and how to fix it

Every figure below is from our own inspection record. The item wording is the condition assessed; the rate is the share of checks of that item marked as findings. Items shown were checked at least 80 times.

Floor hole covers secured and marked

39 findings in 95 checks. 41.1 percent failure rate.

What the inspector is looking at: Whether the cover is fastened down and carries the required sign. 8 CCR 1632(b)(3) calls for letters not less than one inch high reading "Opening--Do Not Remove". Inspectors look for the words, not just the plywood.

How to fix it: Screw or cleat every cover so it cannot be kicked aside, and stencil it before it goes down. An unmarked cover gets moved by the next trade who needs the sheet.

What to document: Log hole locations and cover status by floor. Holes appear and close as the structure progresses, so a one-time list goes stale fast.

Floor holes adequately guarded or covered

55 findings in 139 checks. 39.6 percent failure rate.

What the inspector is looking at: Open penetrations a person can step into. 8 CCR 1632(h) requires floor holes into which persons can accidentally walk to be guarded by a standard railing with standard toeboard on all exposed sides, or by a floor hole cover.

How to fix it: Cover or rail the hole the moment it is created, by whoever creates it. Nearly every finding here is a hole cut by one trade and left for another.

What to document: Make hole creation a logged event. The trade that cuts it owns it until it is covered.

Control lines 6 ft from leading edge for entire length

44 findings in 116 checks. 37.9 percent failure rate.

What the inspector is looking at: Whether the control line is set at the correct distance and runs the full length of the edge. Partial lines and lines set too close both fail.

How to fix it: Measure the offset and run the line end to end. A control line that stops short of the corner is not a control line.

What to document: Record the offset distance and the run length in the daily walk, not just that a line exists.

Safety lines attached securely to substantial anchorages on roof

28 findings in 85 checks. 32.9 percent failure rate.

What the inspector is looking at: What the line is actually tied to. Inspectors pull on it. A rooftop unit, a vent stack or a parapet that has not been engineered as an anchorage does not qualify.

How to fix it: Use a rated anchorage, or where no structural anchor exists, bring in an overhead anchor system rather than improvising. 8 CCR 1670(b)(10) requires 5,000 pounds per attached employee unless the system is engineered to a safety factor of at least two. What to document: Record what each line was anchored to, by location. "Tied off" is not a record.

Controlled access zone defined and access restricted

46 findings in 144 checks. 31.9 percent failure rate.

What the inspector is looking at: Whether the zone exists on the ground rather than on paper, and whether people who are not authorized are being kept out of it.

How to fix it: Define the zone physically and name who is allowed in it before the work starts. This item fails when the plan exists and the site does not match it.

What to document: Name the authorized entrants for each zone and the person controlling access.

Fall arrest or restraint worn when exposed to falls

161 findings in 655 checks. 24.6 percent failure rate.

What the inspector is looking at: Whether the harness is on and connected at the moment of exposure. This item carried 3 serious severity findings in our record, the most of any fall protection condition we track.

How to fix it: Connect before entering exposure, not after arriving at the work position. Most findings here are workers who are equipped, trained and simply not clipped in yet.

What to document: Note the correction and where it happened. Repeat findings at one location usually mean the anchor is inconveniently placed.

Guardrails installed on unprotected surfaces as needed

56 findings in 474 checks. 11.8 percent failure rate.

What the inspector is looking at: Open edges without a rail. This item performs comparatively well because guardrails are visible and hard to forget, unlike a hole in a deck.

How to fix it: Install at the edge as the deck is formed. Passive protection outperforms anything that depends on a worker remembering a step.

What to document: Track edge protection coverage against the deck schedule.

The pattern

Rank these by failure rate and a clear line appears. The things that are visible perform best: guardrails fail at 11.8 percent. The things that are invisible once they look finished perform worst: covered holes fail at 39.6 percent and marking at 41.1 percent.

That is not a training problem or a rules problem. It is that a sheet of plywood lying flat reads as a solved problem to everyone who walks past it, including the person who put it there.

How far can a worker free fall on a personal fall arrest system?

8 CCR 1670(b)(11)(B) requires the system to be rigged such that an employee can neither free fall more than 6 feet, nor contact any lower level. Both conditions have to hold. A system that limits free fall but still lets the worker reach the deck below does not comply.

How strong does a fall arrest anchorage have to be?

8 CCR 1670(b)(10) requires anchorages to be capable of supporting at least 5,000 pounds per employee attached, or under 1670(b)(10)(A) to be designed as part of a complete personal fall arrest system which maintains a safety factor of at least two.

Can two workers share one vertical lifeline?

No. 8 CCR 1670(b)(4) requires that each employee be attached to a separate lifeline, with a limited exception for elevator shafts at 1670(b)(5). This item failed 14.6 percent of the time in our record.
